Case Management / Care Coordination

Case managers are professionals with a specialized body of knowledge and experience related to the care of an older adult. They serve as a guide and resource helping individuals and families develop personalized care recommendations based upon an extensive evaluation of an individual’s physical, psychological, social, and environmental needs.

Our Resource Directory for LGBTQ older adults aims to help people looking for LGBTQ–welcoming services for themselves, a loved one or a client.  The Directory is not meant to endorse or guarantee the sensitivity of any provider. However, we do indicate which organizations and providers report having non-discrimination policies that specifically include sexual orientation (SO) and/or gender identity (GI). The Generations Aging with Pride logo  is indicated next to providers that have had one or more staff members attend our evidence-based training.

Summary:  Lifelong's mission is to empower people living with or at risk (of acquisition or transmission) of HIV, AIDS, and/or other chronic conditions to lead healthier lives. The agency is a Community Health Organization committed to assisting people living with chronic conditions that interfere with their quality of life, independence, and ability to fully participate in their community.
Founded in 1983, Lifelong was forged out of the HIV/AIDS crisis—providing care, prevention and quality of life services to all individuals, regardless of age. Today, our HIV medical case management, wellness advocates and prevention services teams are trained to assist clients in: identifying and accessing resources; coordination of medical and treatment adherence support services; HIV counseling and testing, as well as pre-exposure prophylaxis (or PrEP / Truvada for HIV-negative folks); dental care; health insurance navigation and enrollment; financial resources; food and nutrition; housing and more.

Lifelong’s food and nutrition program—Chicken Soup Brigade—is unique in our region as being the only clinically-focused, diagnosis-specific, meal and grocery operation. Lifelong also provides a wide array of other supportive, wrap-around health-related services to communities that face significant health disparities. Other conditions our case management, prevention, and food programs assist with or aid in the provision and coordination of services include: Diabetes, cardiovascular disease, renal disease, cancers, and multiple sclerosis.

We further serve as a community-based, policy and advocacy agency on issues related to health.
Lifelong currently operates or coordinates services in 12 counties in Washington State: King, Snohomish, Whatcom, Skagit, Island, San Juan, Cowlitz, Wahkiakum, Clallam, Jefferson, Grays Harbor, and Mason.

Summary: SMH's mission is to strengthen our community and improve the lives of our clients by delivering excellent health and human services tailored to meet their needs. Older Adult Services (OAS) provides managed behavioral health care to seniors who have emotional or psychological problems or who are experiencing difficulties with life-impacting events related to the process of aging. OAS staff believe that aging is a positive developmental state and, to that end, insure that the individual client is empowered to choose services that are age appropriate and meet his/her individual needs: OAS provides a variety of services to meet the varied levels of care and respond to the multitude of complex problems. OAS provides counseling, psychiatric evaluations, medication management, and intensive case management. We work closely with the Aging & Adult Services, DSHS, and other geriatric providers. OAS has four locations in King County (Capitol Hill, Bellevue, Tukwila and Auburn).
